Physical Wellness Program Overview
- Prospective Physical Wellness Client contacts Martha’s Hands and reviews information.
- Interested Clients will complete necessary paperwork through Martha’s Hands.
- Clients will work with Martha’s Hands to set up Initial Wellness Evaluation.
- Wellness Coach will evaluate Client and discuss Wellness goals.
- Wellness Coach will provide personalized Wellness Plan and recommended Wellness Program.
- Client finalizes Wellness Program Schedule to meet their personalized Wellness Goals.
Physical Wellness Program Questions & Answers
Who is Eligible? Clients who are generally healthy. If a Client has a chronic illness, we would determine if the Client would benefit from a Wellness Program with no increased health risk.
Who provides the services? Our Wellness Coaches are movement specialists with the expertise to assist in the Client’s ability to maintain or restore their functional potential in the home and community.
Do I need to have any special equipment? No, the Wellness Coach will bring anything needed for your Wellness Visits. The strategy is to enhance your Physical Wellness within your home environment.
How much does it cost? Clients will review their recommended Wellness Program and choose a Bundle of Visits to accomplish their Wellness Goals. Our Bundle Pricing Sheet will communicate our current Bundle Pricing.
Is there a cost for the Initial Wellness Evaluation? Yes, your Initial Wellness Evaluation includes a Capability Test conducted by our Wellness Coach. After the Wellness Evaluation, our Wellness Coach will create a customized Wellness Program to meet your Wellness Goals. It is your decision on how to proceed.
Does Medicare cover this service? Our Physical Wellness Program is Private Pay.
Does Long Term Care Insurance cover this service? Typically, LTC Insurance pays for assistance with daily activities. It usually requires assistance with multiple ADLs. Check with your LTC Insurance carrier. If it does reimburse you for Wellness Services, we can assist you with submitting invoices and clinical documentation.
What is the Scheduling/Cancellation Policy? You will be coordinating an on-going Visit Schedule with our Wellness Coach. Please cancel with 24-hour notice out of respect to the Wellness Coach’s time.
How does Billing work? We email you invoices every two weeks for your review. After five days, if you do not have any questions or concerns regarding the invoice, we will then charge your credit card or process an automatic checking withdrawal (whichever method you prefer).
